I understand that establishing an Easy Lab Tech, LLC agent account does not automatically guarantee that an insurance carrier will reimburse Easy Lab Tech, LLC for services. Billing responsibility and authorization must be established before services are performed. I agree to provide accurate information and understand that Easy Lab Tech, LLC may contact my agency or the applicable carrier/paramedical network to confirm billing authorization.
